fix(a11y): restore keyboard focus when closing notices - #8464
Move keyboard focus into the notice panel's Close control when the user opens it.
Restore focus to the bell when closing, or to the prior app control when dismissing the final notice removes that bell.
Use a stable app-focus fallback only when the previous control no longer exists.
Preserve external focus if notices disappear while the user is working elsewhere. No announcement/provider/runtime changes.
Add four focused keyboard/focus regression cases before the implementation.
Problem
The notice panel is a non-modal disclosure that could open without placing focus inside its content, then disappear after dismissing the final notice without restoring a valid focus destination. This leaves keyboard and screen-reader users without a predictable navigation position. The existing Escape-to-close behavior does not resolve focus restoration.
Solution
Track the trigger, panel-close control, and previous in-app focus target using refs; focus only on the actual open transition. On close, restore focus only when it was still inside the notice subtree. If the bell disappears with the final notice, return to the previously focused still-connected app control, or a stable [data-app-focus-fallback] element. This is an extraction from the fork's UI work, without its unrelated announcement subsystem.

impact: exact-head hosted CI and 100% diff coverage passed; manual keyboard acceptance remains recommended during review.
Behavior Changes
Intended behavior change: predictable focus entry and restoration for the notice disclosure.
User-visible effect: a keyboard user can dismiss the panel without losing place.
Parity Contract
Legacy behavior preserved: same bell, severity badge, notice actions, Escape key, dismissal persistence and position.
Guard/fallback/dispatch parity: regressions cover normal close, last-notice dismissal, removed former control, and notice disappearance while focus is outside.
Duplicate / Superseded PR Handling
Duplicate PR(s): none found for notice focus restoration.
Canonical PR: this focused contribution.
Resolution: extracted only the UX/accessibility change; announcement identity isolation remains separately owned.
Summary by CodeRabbit
Accessibility
Improved focus handling in the notice center: opening it moves focus to the close button, and closing it returns focus to the bell or an appropriate app control.
When notices disappear, focus is preserved if it was outside the notice center.
If the previously focused app control is no longer available, focus moves to a fallback within the app.
NoticeCenter now moves focus to its close button when it opens. When it closes after containing focus, it restores focus to the trigger, a connected previously focused element, or the app fallback. Tests cover these paths and preserve outside focus when notices disappear.

Summary
Focus handling and verification app/src/App.tsx, app/src/components/notices/NoticeCenter.tsx, app/src/components/notices/__tests__/NoticeCenter.test.tsx
The app shell provides a focus fallback. Opening the panel focuses its close button. Closing or removing notices restores focus to the trigger, a connected prior-focus element, or the fallback when appropriate. Tests cover these cases and verify that outside focus remains in place.
The feature remains broadly usable, but some notice-dismissal paths can lose predictable keyboard focus. Fix the fallback handling before relying on this behavior for accessibility.

Pull request purpose (title): fix(a11y): restore keyboard focus when closing notices. This pull request, titled fix(a11y): restore keyboard focus when closing notices, makes the non-modal NoticeCenter disclosure predictable for keyboard users: focus moves into the panel's Close control when the panel opens, returns to the bell or the previously focused still-connected app control on close, falls back to a stable page-level element when that control is gone, and is deliberately not stolen when the user has already moved focus elsewhere. The latest-push-delta section is not available: no trusted base-to-head diff comparison exists, so no push delta is reported and nothing in this summary should be read as one.

What changed
app/src/App.tsx marks the AppShellDesktop root div with `data-app-focus-fallback` and `tabIndex={-1}`, giving the focus-restore effect a real page-level target to fall back to. NoticeCenter.tsx keeps the ref set for the trigger and Close control (`triggerRef`, `closeRef`, plus `panelRef`) alongside `wasOpenRef`, `previousFocusRef`, and `containsFocusRef`, and its effect keyed on `[open, notices.length]` focuses the Close control on the open transition and, when closing while focus was inside, first checks `document.activeElement` so that focus already moved to another app control is left alone rather than being pulled back to the bell; otherwise it restores to `triggerRef.current ?? (previous?.isConnected ? previous : document.querySelector<HTMLElement>('[data-app-focus-fallback]'))`. The container's `onFocusCapture`/`onBlurCapture` handlers record the last outside focused element and track whether focus is inside the notice subtree. NoticeCenter.test.tsx imports `act` and `dismissUserError`, wraps the tree with a `[data-app-focus-fallback]` div containing a 'Resume work' button, and adds five focus tests covering open-focus plus restore-to-bell, restore after dismissing the last notice, fallback when the previous control was removed, preserving outside focus when notices disappear, and not stealing focus after a focused notice disappears while another remains.
Features
Added — Focus entry into the notice panel's Close control on open: Keyboard and screen-reader users land inside the non-modal disclosure when it opens, giving a predictable navigation position. (app/src/components/notices/NoticeCenter.tsxexport default function NoticeCenter() {)
Added — Focus restoration to the bell or the prior in-app control on close: Closing the panel returns focus to the trigger, or to the previously focused still-connected app control, so users do not lose their place. (app/src/components/notices/NoticeCenter.tsxexport default function NoticeCenter() {)
Added — Page-level `[data-app-focus-fallback]` target in the App shell: Provides a stable focus destination when the bell disappears with the final notice and the previously focused control is no longer connected, so focus is not dropped. (app/src/App.tsxexport function AppShellDesktop() {)
Added — Fallback only when the previous control is unmounted: The effect prefers the trigger, then the still-connected previous control, and only resorts to the App-shell fallback, avoiding an unnecessary jump to a page-level element. (app/src/components/notices/NoticeCenter.tsxexport default function NoticeCenter() {)
Added — Preservation of outside focus when notices disappear: If notices vanish while the user is working elsewhere, or a focused notice unmounts leaving stale blur state, the active-element check keeps focus where the user put it instead of restoring the bell. (app/src/components/notices/NoticeCenter.tsxexport default function NoticeCenter() {)
Tests
test_addition — Covers restoring app focus to the prior control after dismissing the last notice removes the panel.: The tests lane, which answered, reported this as part of the five focus tests and found no defect. Execution is not asserted by trusted check evidence here. (app/src/components/notices/__tests__/NoticeCenter.test.tsxdescribe('NoticeCenter', () => {)
test_addition — Covers falling back to persistent app content when the previously focused control was unmounted.: The tests lane, which answered, reported this fallback branch is covered against real DOM focus; the critique lane's earlier Likely-confidence note that the fallback was asserted against a node the app never renders is addressed by the App.tsx fallback now present in this revision, and critique retained no findings here. (app/src/components/notices/__tests__/NoticeCenter.test.tsxfunction renderCenter(descriptors: UserErrorDescriptor[] = []) {, app/src/App.tsxexport function AppShellDesktop() {)
test_addition — Covers preserving outside focus when notices disappear while the user is elsewhere.: The tests lane, which answered, reported the negative assertion here is of the kind that fails if the new effect wrongly restores focus, and found no defect. (app/src/components/notices/__tests__/NoticeCenter.test.tsxdescribe('NoticeCenter', () => {)
test_addition — Covers not stealing focus after a focused notice disappears while another notice remains, with focus then moved outside before the final dismissal.: The tests lane, which answered, reported the focus change is covered by behavior-level assertions on specific focus outcomes rather than mocks. Execution is not asserted by trusted check evidence here. (app/src/components/notices/__tests__/NoticeCenter.test.tsxdescribe('NoticeCenter', () => {)
test_harness_change — Adds a `[data-app-focus-fallback]` wrapper containing a 'Resume work' button to the test tree, matching the App-shell fallback the effect queries.: The tests lane, which answered, reported no test defect and noted the App.tsx fallback target the effect queries is added in the same diff. (app/src/components/notices/__tests__/NoticeCenter.test.tsxfunction renderCenter(descriptors: UserErrorDescriptor[] = []) {, app/src/App.tsxexport function AppShellDesktop() {)

critique
Conclusion: Success
Scope reviewed: all assigned evidence
Positive: The critique lane answered and retained no findings this round: it reported the focus-tracking refs are updated through capture handlers and the restore effect guards against stale blur state, with no concrete input on which the new code misbehaves.
Lane summary: The change adds focus management to NoticeCenter (focus the close button on open, restore focus to the bell or a fallback on close) and marks the App shell root with data-app-focus-fallback, with tests for each path. The focus-tracking refs are updated through capture handlers and the restore effect guards against stale blur state; I found no concrete input on which the new code misbehaves, and the new tests exercise the behaviours the diff adds. _The code index is behind this pull request (indexed at `03e62a25930d`), so retrieved context may be out of date._
security
Conclusion: Success
Scope reviewed: all assigned evidence
Positive: The security lane answered and found no concrete defect: no untrusted input reaches a sink, no auth or network change, and the focus logic degrades to doing nothing rather than misbehaving for the inputs it handles; it noted the forge API search was unavailable so its verdict rests on the diff and provided repository context.
Lane summary: Focus-management change in NoticeCenter plus a tabIndex fallback attribute in App.tsx; I reviewed the effect's focus restoration logic and the new tests. I could not read the surrounding files (the forge API search is unavailable), so my verdict is based on the diff and the provided repository context. No concrete defect found: no untrusted input reaches a sink, no auth or network change, and the focus logic degrades to doing nothing rather than misbehaving for the inputs it handles. _The code index is behind this pull request (indexed at `03e62a25930d`), so retrieved context may be out of date._
tests
Conclusion: Success
Scope reviewed: all assigned evidence
Positive: The tests lane answered and found no test defect, reporting five behavior-level tests each asserting a specific focus outcome against real DOM focus, with the negative outside-focus assertions of the kind that fail on a wrong restore; it noted it could not mutate the code to confirm each test fails on regression.
Lane summary: The focus-management change in NoticeCenter is covered by five behaviour-level tests that each assert a specific focus outcome (open moves focus to close, close and last-notice dismissal restore to the bell, unmounted previous control falls back to `[data-app-focus-fallback]`, and outside focus is preserved), and the App.tsx fallback target the effect queries is added in the same diff. Nothing here asserts on mocks or on 'something rendered'; I found no test defect worth reporting. I could not mutate the code to confirm each test fails on regression, but the negative assertions (focus stays on the outside control) are of the kind that do fail if the new effect wrongly restores. _The code index is behind this pull request (indexed at `03e62a25930d`), so retrieved context may be out of date._

description
Conclusion: Success
Scope reviewed: all assigned evidence
Lane summary: The diff implements what the description claims: focus moves into the panel's Close control on open, restores to the bell or a previously focused still-connected app control on close, and uses the added `[data-app-focus-fallback]` wrapper in App.tsx as the last resort. The four regression tests in NoticeCenter.test.tsx match the described cases. The change is presentation-only, adds no RPC, i18n or state-layer changes, and I found no defect in the changed lines — it looks safe to merge. I could not run the test suite locally, so correctness of the focus assertions rests on reading the code and the hosted CI result the author cites. _The code index is behind this pull request (indexed at `03e62a25930d`), so retrieved context may be out of date._

Restore focus when the final notice is removed while the bell is focused and the panel is closed.
The final-removal branch at NoticeCenter.tsx:125 requires wasOpenRef.current. That ref equals the previous visible value, and visible requires open. While the panel is closed, the branch never runs. If the bell has focus and the last notice is removed, the bell unmounts with NoticeCenter and focus falls to document.body. The shell fallback is never selected.
The removal does not need the panel. useAppNotices.ts:147 dispatches resolveUserError, and Skills.tsx:796 dispatches dismissUserError, and neither requires the panel to be open.
The bell is inside the notice subtree, so the stated contract (restore focus when it stayed in the subtree) applies to this case.
The change keeps the existing open-panel behavior. It restores focus on the closed-panel path only when the last notice is removed. It does not restore focus when other notices remain.

Minor · Do not restore focus to document.body. · NoticeCenter.tsx:125-131
When focus starts on document.body and the user opens the notice panel, onFocusCapture stores document.body in previousFocusRef. After the final notice is dismissed, the bell ref is cleared and the connected body element wins over [data-app-focus-fallback]. The effect then calls document.body.focus(), so focus does not move to a usable app target.
Exclude document-level elements when selecting the previous target. This is separate from the closed-panel wasOpenRef branch.

The test focuses the first notice, dismisses it, then focuses Resume work before asserting. A regression that calls focus() during the first dismissal can therefore pass. Add an immediate spy assertion before changing focus.
