Do not treat an unpublished configuration as initialized.

If init({shm_name = "missing"}) fails, the local conf becomes {}, but _M.conf remains unset. A later init() matches conf.shm_name == nil and returns _M, nil here without creating a valid public configuration. The v3 request path later reads health_check.conf.retry, which can fail on nil. Build a candidate configuration and publish it only after validation, or require _M.conf == conf before taking this fast path.
