Summary

Screen context (window screenshot + Vision OCR) kept restarting or invalidating for the same focused field while the user typed. Each time it re-ran capture and OCR, hid the visible suggestion, cancelled work in flight, and changed the llama prompt head, which forced a full re-prefill. There were four independent causes:

  1. Placeholder flips read as navigation (e09e63c). The Claude desktop composer reports AXPlaceholderValue only while it is empty ("Type / for commands."). Since Integrate autocomplete improvements while preserving upstream lifecycle #832 the placeholder is read live on every poll and is part of both FocusedInputPollingSignature and FocusedInputSessionIdentity, so every empty↔text transition opened a new focus session. Now a placeholder that disappears is never evidence of navigation. One that appears counts only when it differs from the placeholder this session already showed, which the remembered signature keeps while it is hidden. The placeholder is dropped from FocusedInputSessionIdentity, because the tracker's focusChangeSequence already carries a real change. Prompts still read the live placeholder.
  2. ⌘A tore down screen context (9a7558e). Key, scheduling and reconcile paths reach the full disablePredictions, which cancelled visual context for the transient "Text is currently selected." state. Only the focus-change path preserved it. SuggestionAvailabilityEvaluator.shouldKeepVisualContextWhileDisabled now keeps the session when the reason is the field's own capability state and capture is still allowed.
  3. The focused field's own text changed every refresh (4436591). OCR lines inside the focused element's frame are dropped (that text is already the caret prefix), and its pixels are left out of the OCR cache hash, so a refresh where only the typing changed reuses the cached extraction.
  4. OCR jitter on unchanged pixels counted as a change (f482539). Examples are truncated titles ("sessi"/"sessic"), spinners, and labels that come and go. VisualContextExcerptStability treats near-identical excerpts as unchanged and keeps the current text byte-identical.

FocusTracker also logs which identity facts opened a new focus session (fact names only, no values).

Validation

  • Full unit suite on the merged branch: xcodebuild test -workspace build/cotabby-dependencies/Cotabby.xcworkspace -scheme Cotabby -destination 'platform=macOS' -derivedDataPath build/DerivedData CODE_SIGNING_ALLOWED=NO → 2814 tests, 0 failures, 16 skipped.
  • Live check: Cotabby Dev Release build launched with -cotabby-debug, typing in the Claude desktop composer, llama engine. Metrics come from cotabby.jsonl:
no fix (24 min) placeholder fix only (20 min) all four fixes (1.1 min)
"screen context changed" resets / captures 53 / 87 (61%) 145 / 159 (91%) 7 / 20 (35%)
resets with no key in the previous 3.5 s 4 38 0
"focused field changed" pipeline resets 16 6 0
same-field visual-context session restarts 17 28 3

The combined run is short and heavy on select-all/clear, so read its column as a trend. In that run, 9 refreshes kept the excerpt as OCR jitter, and 7 of 18 extractions were served from the OCR cache.

  • Not run: swiftlint --strict (not installed locally). Not run: the model-backed eval suites. Prompt rendering is unchanged, and both suites inject fixed visualContextSummary strings, so the capture-side filtering does not reach them.

Risk / rollout notes

  • Navigation detection (placeholder). Going from an empty field showing a placeholder to a field that already has text, with the same title, URL, frame and role, is no longer a session boundary. Changed screen text remains the fallback signal. A field first seen with text and then cleared still opens one new session; this is kept deliberately so that a switch from a drafted chat to an empty one is still detected.
  • Known remaining restart. The Claude composer swaps its placeholder between "⏎ to send, ⌘⏎ to queue for later." (while a reply streams) and "Type / for commands.". That counts as a new session, but only while the field is empty, so no suggestion is cut off.
  • Excerpt stability. Small changes (no new line of real text, under about 5% of the excerpt) keep the previous excerpt, so screen context can lag a tiny on-screen change by one refresh.
  • project.pbxproj gains FocusedFieldScreenRegion, VisualContextExcerptStability and their tests.

Summary by CodeRabbit

  • Bug Fixes
    • Typing in a focused field no longer causes its placeholder disappearing to be mistaken for navigation, helping preserve the current session.
    • Visual context excludes text from the focused field and avoids unnecessary updates when screen text changes only due to OCR variation.
    • Updates to the screen outside the focused field are still detected, while changes caused only by typing do not trigger unnecessary OCR processing.
    • Visual context can remain available during certain temporary prediction-disabled states when capture is permitted.
    • Missing text after the caret is now distinguished from an empty field tail, improving context handling when field text is incomplete.

Summary

This PR keeps screen context steady while the user types.

  • Typing no longer makes a hidden placeholder look like a new field.
  • A focused field keeps its screen context through temporary prediction pauses.
  • Screen context now leaves the focused field's own text out of its change signal.
  • Small OCR differences no longer replace the screen context in use.

The Claude desktop composer reports AXPlaceholderValue ("Type / for
commands.") only while it is empty: the value lingers for a poll after
the first typed character, then disappears, and returns when the field is
cleared. Since the placeholder became a live per-poll fact in both
FocusedInputPollingSignature and FocusedInputSessionIdentity, each of
those transitions read as navigation. FocusTracker advanced
focusChangeSequence, so the visual-context session restarted (a new
screenshot and Vision OCR, discarding the in-flight one) and the
suggestion pipeline reset as if the field had changed.

In a 24-minute dev-log run, 17 of 26 visual-context session starts were
such same-field restarts, 7 captures were cancelled mid-OCR, and 16
"focused field changed" resets fired, all on empty <-> text transitions.

- FocusedInputPollingSignature.continuation(of:): a disappearing
placeholder is never evidence of navigation; an appearing one is only
when it differs from the placeholder this session already showed,
which the remembered signature keeps while the host hides it.
- FocusedInputSessionIdentity drops the placeholder; the tracker's
sequence already carries a genuinely different one. Prompts still read
the live placeholder, so prompt content is unchanged.
- FocusTracker logs which identity facts opened a new focus session.

Pressing Cmd+A in a chat composer publishes "Text is currently selected."
until the next keystroke replaces the selection. The focus-change path
already treats that as transient and keeps the visual-context session
(disablePredictionsPreservingVisualContext), but key, scheduling and
reconcile paths reach the full disablePredictions, which always cancelled
it. The next keystroke then started a new screenshot + Vision OCR and a
new prompt head. A live dev-build check showed 2 of the 4 same-field
restarts left after the placeholder fix came from this path.

SuggestionAvailabilityEvaluator.shouldKeepVisualContextWhileDisabled
keeps the session only when the disable reason is the field's own
capability state, the field is present and not secure, and capture is
still allowed; environment reasons (off, paused, disabled app or site,
permissions, fast mode) still end it. disablePredictions consults it.

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
Every 3 s refresh re-OCRs the focused window. The field being typed into is
part of that window, so its text changed the excerpt on almost every refresh,
and SuggestionCoordinator treats a changed excerpt as navigation: it retires
the suggestion, and the new prompt head forces a full llama re-prefill.

FocusedFieldScreenRegion places the focused element's own frame
(elementFrameRect, not the widened inputFrameRect) inside the capture. OCR
lines centred in it are dropped (the field's text is already the caret prefix,
and the text-based echo strip misses the line being typed because the
screenshot lands a keystroke after the snapshot), and its pixels are left out
of the OCR cache's pixel hash. A refresh whose only change is typing now reuses
the cached extraction and yields a byte-identical excerpt, so no reset fires
and Vision does not run. Any change outside the field still misses the cache
and reaches the excerpt. The field rectangle is mixed into the cache key.

The two llama eval suites inject fixed visualContextSummary strings, so their
prompts are unchanged by construction.

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
Vision reads unchanged pixels differently between captures: truncated
sidebar titles flicker ("sessi"/"sessic"), spinners read as "8"/"88", a line
splits in two, labels and digit runs come and go, timers tick. Every such
refresh changed the excerpt, and SuggestionCoordinator treats a changed
excerpt as navigation, so the suggestion vanished and came back every 3 s even
while the user was not typing, and each refresh replaced the llama prompt head.

VisualContextExcerptStability compares the refreshed excerpt with the one in
use, line by line on a key without case, digits or one-letter fragments. Lines
that are the same line read twice (truncation, a few characters, one word) pair
up; the change is material when any unpaired line is real text (three words,
or CJK text of comparable length) or the differences add up to more than about
5% of the excerpt. VisualContextCoordinator keeps the current excerpt
byte-identical when the change is not material, so no reset fires and the
prompt head stays put. A new message or another conversation still counts.

Calibrated on the 21:56-22:17Z dev-log run: of 102 excerpt changes seen in
prompts, 42 would still reset when compared against the kept excerpt.

Walkthrough

Focused-input identity now handles disappearing placeholders and unavailable trailing text. Visual-context capture excludes focused-field content from OCR and cache hashing, retains excerpts across non-material OCR changes, and can remain active for eligible disabled prediction states.

Changes: Focused Input and Visual Context

Session identity no longer includes the field placeholder. Polling retains a previously observed placeholder when it disappears. Focus snapshots track unavailable trailing text and preserve that state through resolution and snapshot copying.
Focused-field OCR exclusion and caching

Screenshot capture provides an optional focused-field region. OCR lines within that region are excluded, and pixel hashing skips its pixels while including the region bounds. Tests cover geometry, filtering, cache reuse, and extraction behavior.
Excerpt stability and refresh behavior
Cotabby/Support/Context/VisualContextExcerptStability.swift, Cotabby/Services/Visual/VisualContextCoordinator.swift, related tests, Cotabby.xcodeproj/project.pbxproj, ARCHITECTURE.md, SOURCE_LAYOUT.md, AGENTS.md
The coordinator retains its current excerpt when refreshed text is not materially different. It notifies only when adopted text changes. Tests cover OCR variation and meaningful screen changes.
Visual context while predictions are disabled
Cotabby/Support/Suggestion/Request/SuggestionAvailabilityEvaluator.swift, Cotabby/App/Coordinators/Suggestion/SuggestionCoordinator+Prediction.swift, related tests
Visual context remains active for qualifying field-specific disabled states when capture is permitted. The evaluator rejects secure or missing context and capture-disabling conditions.

Review on FuJacob#884: VisualContextExcerptStability pairs every unmatched new
line with every unmatched old line, and each attempt could fill a full
character edit table. Measured with swiftc -O: real refreshes took 0.45 ms
mean and 1.6 ms worst, but 40 near-identical code lines with small misreads
took about 55 ms, on the main actor.

Lines are now prepared once and compared as Unicode scalars; the edit
distance is banded (only cells within the limit of the diagonal), and each
pairing attempt only has to beat the best pairing found so far. A total
budget of 200,000 cells per call backs that up: past it the change counts
as material, so that refresh resets as every refresh did before the rule.
The adversarial cases now take about 4 ms, half of it line parsing.

Verdicts are unchanged on the 102 consecutive excerpts of the dev-log run
and on 975 pairs up to ten refreshes apart; no real pair needed more than
about 1,900 cells.

Review on FuJacob#884: dropping every OCR line inside the focused field assumed
its text already reaches the prompt, but focus capture keeps only 4,096
UTF-16 units on each side of the caret. In a long document, visible lines
farther away are text the prompt never received, and they used to survive
the text-based echo strip.

When either side of the snapshot may have been cut to that window, the
field region shrinks to the caret's own line band: the line being typed,
which is always inside the window and is the one the echo strip misses.
The editor's other lines go through the echo strip as before. Fields whose
whole text arrived, every chat composer among them, keep the whole-field
exclusion. FocusedInputSnapshot gains trailingTextMayBeTruncated to match
precedingTextMayBeTruncated.

Review on FuJacob#884: a host may answer AXPlaceholderValue with "" instead of
dropping the attribute once typed text hides the placeholder. The signature
kept "", which differs from the placeholder the session showed, so the poll
opened a new focus session and cancelled the generation in flight.

FocusedInputPollingSignature now reads an empty or whitespace-only
placeholder as none, the same way the prompt side already does
(SurfaceContextComposer.sanitizedPlaceholder).

The excerpt comparison paired "Error saving" with "Error saving draft" as one
misread word, so the new line never reached the leftover check and the refresh
was kept. A pairing is now material when the new line is the old one plus one
word at its end and that word made it a real line. Misreads change words
inside a line, so "OpenAI t 1 Chromium" / "OpenAI tEN Chromium" is still
jitter. A line OCR had split and reads whole again ("Opus Extra" + "Auto") is
exempt: the added word is still on screen as its own leftover line.

Checked on the recorded dev-log run: all 102 consecutive and 975 wider
verdicts are unchanged.

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
A failed AXStringForRange read of the text after the caret became "", so a
multi-line field whose text continued past the caret looked complete and the
screenshot region excluded the whole field, dropping visible text the prompt
never received. The text stays as it was: Chromium fails this read in every
multi-block contenteditable, and falling back to AXValue would pair the value
with a range-space caret. The failure is recorded instead as
`trailingTextUnavailable` on the snapshot (cleared when restoringBlockBreaks
rebuilds the text from the value), and `trailingTextMayBeTruncated` honours
it, so only the caret's line band is excluded. The text-marker path had the
same fallback and sets the same flag.

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
Vision can split one visual line at a different word on each capture:
"Error saving" + "Draft note" one time, "Error saving draft" + "note" the
next. The grown-into-a-real-line guard from 84ed961 exempted only a word that
had stood as a line of its own, so this reread reset the suggestion. The added
word is now exempt when it began any old line the refresh changed (left over
after exact matching). A word that only matches the start of an unchanged line
still counts as new text.

All 102 consecutive and 975 wider verdicts from the recorded run are unchanged.

Confirm that the word moved from the matched line.

An unrelated changed line can start with added. For example, Error saving\nDraft note changing to Error saving draft\nDraft saved is treated as non-material, although draft remains on the second line. The coordinator then retains the old excerpt and sends no context-change callback.

Match the source line’s remainder against the candidate before treating the word as moved. Also exclude the paired current line itself as a source.

…yping (#32)

Port of upstream FuJacob#884. Four causes made the same field's screen context re-capture,
hide the visible suggestion and change the llama prompt head (full re-prefill):
- a placeholder that disappears on the first typed character read as navigation;
- Cmd-A ("Text is currently selected.") tore down the field's visual-context session;
- the focused field's own text changed every OCR refresh (its lines are now dropped and its
  pixels left out of the OCR cache hash);
- OCR jitter on unchanged pixels counted as a change (VisualContextExcerptStability).

Ghostype changes on top: the new keep-session gate also honours Complete in Coding Agents and the
Code Editors list; VisualContextExcerptStability replaces Ghostype's older ScreenExcerptChangePolicy,
which kept a stale excerpt when a new message arrived; two functions split for strict SwiftLint.
