Run OCR one pass at a time so a stuck Vision call cannot freeze the menu - #885
After several hours with screen context on, the menu bar popover stopped closing on an outside click and its buttons (Quit included) stopped responding. The main thread was idle. A sample of the stuck app showed 44 threads parked in ScreenTextExtractor.extractText → VNImageRequestHandler.perform, all queued behind one Vision request that was itself parked forever on a semaphore inside TextRecognition. libdispatch reported "Dispatch Thread Soft Limit: 90 reached". With no workers left, AppKit's activation callbacks never ran: Accessibility still reported Cotabby as frontmost while another app was active, so the popover never resigned key.
The cause is that each OCR pass ran blocking Vision work on its own DispatchQueue.global worker, with no limit on concurrency and no timeout. OCRWorkGate now runs every pass on one serial queue:
A wedged pass costs one thread, not one per later pass.
A caller waits at most 60 s, or until its task is cancelled, never forever.
While the running pass is past its deadline, new callers are refused at once (ScreenTextExtractionError.ocrUnavailable, which screen context already reports as unavailable context) instead of queueing behind it.
Queued work whose caller already gave up is skipped when its turn comes.
perform is synchronous, and Vision fills request.results before it returns. The pass now reads the results directly and no longer uses a completion handler, so the old OCRContinuationResumer double-resume guard (#698) is no longer needed: three-way settlement (work, timeout, cancel) lives in the gate.
The 60 s timeout is deliberately loose. The first accurate pass in a process loads Vision's models; the existing blank-image test took 31 s on unmodified code on a busy machine. The starvation guard is the serial queue, not the timeout.
Validation
xcodebuild test with -only-testing:CotabbyTests/ScreenTextExtractorTests, ScreenshotContextGeneratorTests, VisualContextCoordinatorTests on this branch (upstream main + this commit): ** TEST SUCCEEDED **, 35 tests, 0 failures.
New gate tests: returns the result; times out a caller whose work is stuck; refuses fast while a pass is wedged and admits work again once it returns; skips queued work whose caller timed out; cancellation ends the wait.
swiftlint lint --strict on both changed files: no violations.
Diagnosis came from a live stuck process (sample, CGWindowListCopyWindowInfo, AX AXFrontmost). The same fix is installed on my daily build. It has not yet run long enough to show the hang is gone in practice; it took about 10 hours to build up.
Risk / rollout notes
OCR passes no longer run concurrently. A pass that arrives while another is running waits for it (bounded by the timeout). Screen context is refreshed per field, not per keystroke, so this should not be noticeable.
If Vision wedges for good, screen context reports unavailable for the rest of the session instead of hanging the app.
Every OCR pass ran VNImageRequestHandler.perform on its own GCD worker with
no concurrency bound and no timeout. One Vision request wedged inside
TextRecognition; each later pass queued behind it and parked another thread.
After ~10 hours 44 were parked, libdispatch hit its 90-thread soft limit, and
AppKit's activation callbacks stopped getting threads: the menu bar popover
never learned the app was deactivated, so it would not close and Quit did
nothing.
OCRWorkGate runs every pass on one serial queue, so a wedged pass costs one
thread. Callers wait at most 60 s or until their task is cancelled, are
refused at once while the running pass is past its deadline, and queued work
whose caller already gave up is skipped.

Walkthrough
OCR extraction now uses a shared serial work gate. Callers can time out or cancel their wait, and extraction maps gate and Vision failures to specific errors. Tests cover result delivery, timeout, busy refusal, skipped queued work, and cancellation.

Summary
Gate lifecycle and validation Cotabby/Services/Visual/ScreenTextExtractor.swift, CotabbyTests/Services/Visual/ScreenTextExtractorTests.swift
OCRWorkGate serializes work, handles caller timeouts and cancellation, skips queued work after its caller settles, and rejects new work while an overdue pass remains active. Tests cover these behaviors.
Text extraction integration Cotabby/Services/Visual/ScreenTextExtractor.swift
extractText submits synchronous Vision work through the shared gate. It preserves sorting, confidence, bounds, and text-length handling, and maps gate and Vision errors to extraction errors.

Busy test races its deadline
If the queued work starts late, this test can fail even when the gate works correctly. The caller’s timer starts before the work, but runningDeadline starts inside the work. The first call can therefore time out before runningDeadline expires, so the second call waits and throws .timedOut instead of .busy.
Wait for the work to start and its running deadline to pass before checking refusal. Also release the semaphore in defer so an early failure cannot leave the worker blocked.

Remove the fixed-delay assumption from the busy-refusal test.
OCRWorkGate.run starts the caller timer before queue execution, but sets runningDeadline only when the queued work starts. A 50 ms sleep covers only queue delays up to 50 ms. A longer delay can leave the pass within its deadline, so the second call can queue and later throw .timedOut instead of .busy.
Synchronize with the first work item's start, then use bounded polling with short attempts until run returns .busy. Do not rely on a fixed sleep.

Port of upstream FuJacob#885. Each OCR pass ran blocking Vision work on its own global
dispatch worker with no limit and no timeout; one wedged Vision request parked every later pass
behind it until GCD hit its thread limit and AppKit callbacks (menu popover, Quit) stopped running.
OCRWorkGate runs passes on one serial queue, waits at most 60 s or until cancelled, and refuses
new callers at once while the running pass is past its deadline.
