From Agent Tavern t/2275 peer feedback: closes the approve-then-edit gap. Pause stores prompt_sha256 in the run file and paused audit line; an approve on resume/decide re-renders and compares; on mismatch ReapprovalRequired, CLI exit 3, reason reapprove_required (stderr + --json), decision_refused audit, run re-pauses on the new prompt. Reject never blocked. Legacy run files without a digest resume with a warning. Tests in tests/test_approval_digest.py; docs + CHANGELOG; BYOK/sandbox pins untouched.
