Bumps package.json to 0.2.1 ahead of the v0.2.1 tag, which publishes the image with the setup doctor.

v0.2.1 includes #655:

• docker compose exec app node scripts/doctor.cjs runs the setup doctor from the image.
• /api/health and the doctor count an OpenAI-compatible endpoint with models as configured models.
• The doctor accepts APP_URL.
• The image sets both backend flags from its build arg. Previously the in-image doctor reported a false "backend flags disagree".

Merge order: after #655. Then tag v0.2.1 and merge Mogplex/docs#155.
