  • docs/decisions/README.md maps each former number to its topic record.
  • All links in the docs and decision references in code comments now point at the topic records.
  • The rule in CLAUDE.md/AGENTS.md changes from "never rewrite a superseded decision" to "edit the topic's record and say what changed". README and CONTRIBUTING are updated to match.
  • The records total 1,354 lines, down from 4,569. They keep current rules, the deciding matrices, key measurements, and open evidence gates, and drop narrative, superseded options, and per-milestone test counts.

scripts/check.sh and scripts/check_fast.sh pass. The only code changes are comments.

Each topic record states the current decision; superseded history stays
in git at 3ddc739. Links and code comments point at the topic records,
and the contributor rules now ask for edits to the topic record instead
of a new numbered record per change.
