GitHub issue action settings remain stuck loading in new Issue Alert #122406

Environment

SaaS (https://sentry.io/)

Steps to Reproduce

In Sentry SaaS, navigate to Monitors → Alerts → Create Alert.
Select “Alert on all issues in selected projects” and choose a project.
Configure the trigger “A new issue is created.”
Under “THEN perform these actions,” select “Create a GitHub issue.”
Click “Action Settings.”
Observe that the modal displays an indefinite loading spinner and never loads the repository configuration fields.

A validation/schema error also appeared at the bottom of the alert page indicating that the required repo field was missing, but the repository cannot be selected because Action Settings never finishes loading.

The GitHub integration is installed and the Ticket Rules feature is shown as enabled. Sentry also displays a notice that the GitHub App is not on its latest version; however, updating it may affect our organization-wide installation, so I have not updated it.

Expected Result

The Action Settings modal should load the available GitHub repositories and allow me to select a repository and configure the GitHub issue that will be created by the alert.

Actual Result

The Action Settings modal opens but remains indefinitely on a blank loading screen with a spinner. The repository and other GitHub issue configuration fields never appear, so the alert cannot be completed.

The alert page also displays a raw validation/schema error stating that the required repo field is missing. I cannot provide that field because the Action Settings modal does not load.

Refreshing the page and removing and re-adding the GitHub action did not resolve the problem. Screenshots are attached.
